block: fix blk_queue_end_tag()

Commit 5e081591 "block: warn if tag is greater than real_max_depth"
cleaned up blk_queue_end_tag() to warn when the tag is truly invalid
(greater than real_max_depth).  However, it changed behavior in the tag <
max_depth case to not end the request.  Leading to triggering of
BUG_ON(blk_queued_rq(rq)) in the request completion path:

In order to allow blk_queue_resize_tags() to shrink the tag space
blk_queue_end_tag() must always complete tags with a value less than
real_max_depth regardless of the current max_depth.  The comment about
"handling the shrink case" seems to be what prompted changes in this
space, so remove it and BUG on all invalid tags (made even simpler by
Matthew's suggestion to use an unsigned compare).

All requests of a queue could be merged to other requests of other queue.
Such queue will not have request in it, but it's in service tree. This
will cause kernel oops.
I encounter a BUG_ON() in cfq_dispatch_request() with next patch, but the
issue should exist without the patch.

block/swim3: Locking fixes

The old PowerMac swim3 driver has some "interesting" locking issues,
using a private lock and failing to lock the queue before completing
requests, which triggered WARN_ONs among others.

This rips out the private lock, makes everything operate under the
block queue lock, and generally makes things simpler.

We used to also share a queue between the two possible instances which
was problematic since we might pick the wrong controller in some cases,
so make the queue and the current request per-instance and use
queuedata to point to our private data which is a lot cleaner.

We still share the queue lock but then, it's nearly impossible to actually
use 2 swim3's simultaneously: one would need to have a Wallstreet
PowerBook, the only machine afaik with two of these on the motherboard,
and populate both hotswap bays with a floppy drive (the machine ships
only with one), so nobody cares...

While at it, add a little fix to clear up stale interrupts when loading
the driver or plugging a floppy drive in a bay.

cfq-iosched: fix cfq_cic_link() race confition

cfq_cic_link() has race condition. When some processes which shared ioc
issue I/O to same block device simultaneously, cfq_cic_link() returns -EEXIST
sometimes. The race condition might stop I/O by following steps:

step  1: Process A: Issue an I/O to /dev/sda
step  2: Process A: Get an ioc (iocA here) in get_io_context() which does not
    linked with a cic for the device
step  3: Process A: Get a new cic for the device (cicA here) in
    cfq_alloc_io_context()

step  4: Process B: Issue an I/O to /dev/sda
step  5: Process B: Get iocA in get_io_context() since process A and B share the
    same ioc
step  6: Process B: Get a new cic for the device (cicB here) in
    cfq_alloc_io_context() since iocA has not been linked with a
    cic for the device yet

step  7: Process A: Link cicA to iocA in cfq_cic_link()
step  8: Process A: Dispatch I/O to driver and finish it

step  9: Process B: Try to link cicB to iocA in cfq_cic_link()
    But it fails with showing "cfq: cic link failed!" kernel
    message, since iocA has already linked with cicA at step 7.
step 10: Process B: Wait for finishig I/O in get_request_wait()
    The function does not wake up, when there is no I/O to the
    device.

When cfq_cic_link() returns -EEXIST, it means ioc has already linked with cic.
So when cfq_cic_link() return -EEXIST, retry cfq_cic_lookup().

struct request_queue is allocated with __GFP_ZERO so its "node" field is
zero before initialization.  This causes an oops if node 0 is offline in
the page allocator because its zonelists are not initialized.  From Dave
Young's dmesg:

and __alloc_pages_nodemask+0xb5 translates to a NULL pointer on
zonelist->_zonerefs.

The fix is to initialize q->node at the time of allocation so the correct
node is passed to the slab allocator later.

Since blk_init_allocated_queue_node() is no longer needed, merge it with
blk_init_allocated_queue().

  "This breaks some powerpc platforms at least.  The practice of having
   a node provide an explicit "interrupt-parent" property pointing to
   itself is an old trick that we've used in the past to allow a
   device-node to have interrupts routed to different controllers.

   In that case, the node also contains an interrupt-map, so the node is
   its own parent, the interrupt resolution hits the map, which then can
   route each individual interrupt to a different parent."

Grant says:
  "Ah, nuts, yes that is broken then.  Yes, please revert the commit and
   Rob & I will come up with a better solution.

   Rob, I think it can be done by explicitly checking for np ==
   desc->interrupt_parent in of_irq_init() instead of relying on
   of_irq_find_parent() returning NULL."

Btrfs: remove free-space-cache.c WARN during log replay

The log replay code only partially loads block groups, since
the block group caching code is able to detect and deal with
extents the logging code has pinned down.

While the logging code is pinning down block groups, there is
a bogus WARN_ON we're hitting if the code wasn't able to find
an extent in the cache.  This commit removes the warning because
it can happen any time there isn't a valid free space cache
for that block group.

To prevent an NFS server from being used to create a directory loop in an NFS
superblock on the client, the following patch was committed:

Subject: fix loop checks in d_materialise_unique()

This causes ELOOP to be reported to anyone trying to access the dentry that
would otherwise cause the kernel to complete the loop.

However, no indication is given to the caller as to why an operation that ought
to work doesn't.  The fault is with the kernel, which doesn't want to try and
solve the problem as it gets horrendously messy if there's another mountpoint
somewhere in the trees being spliced that can't be moved[*].

[*] The real problem is that we don't handle the excision of a subtree that
gets moved _out_ of what we can see.  This can happen on the server where a
directory is merely moved between two other dirs on the same filesystem, but
where destination dir is not accessible by the client.

So, given the choice to return ELOOP rather than trying to reconfigure the
dentry tree, we should give the caller some indication of why they aren't being
allowed to make what should be a legitimate request and log a message.

We've been hitting BUG()'s in btrfs_cont_expand and btrfs_fallocate and anywhere
else that calls btrfs_get_extent while running xfstests 13 in a loop.  This is
because fiemap is calling btrfs_get_extent with non-sectorsize aligned offsets,
which will end up adding mappings that are not sectorsize aligned, which will
cause problems in some cases for subsequent calls to btrfs_get_extent for
similar areas that are sectorsize aligned.  With this patch I ran xfstests 13 in
a loop for a couple of hours and didn't hit the problem that I could previously
hit in at most 20 minutes.  Thanks,

using our normal prepare_pages stuff, which means I of course forgot to do
things like set the pages extent mapped, which will cause us all sorts of

Btrfs: wait on caching if we're loading the free space cache

We've been hitting panics when running xfstest 13 in a loop for long periods of
time.  And actually this problem has always existed so we've been hitting these
things randomly for a while.  Basically what happens is we get a thread coming
into the allocator and reading the space cache off of disk and adding the
entries to the free space cache as we go.  Then we get another thread that comes
in and tries to allocate from that block group.  Since block_group->cached !=
BTRFS_CACHE_NO it goes ahead and tries to do the allocation.  We do this because
if we're doing the old slow way of caching we don't want to hold people up and
wait for everything to finish.  The problem with this is we could end up
discarding the space cache at some arbitrary point in the future, which means we
could very well end up allocating space that is either bad, or when the real
caching happens it could end up thinking the space isn't in use when it really
is and cause all sorts of other problems.

The solution is to add a new flag to indicate we are loading the free space
cache from disk, and always try to cache the block group if cache->cached !=
BTRFS_CACHE_FINISHED.  That way if we are loading the space cache anybody else
who tries to allocate from the block group will have to wait until it's finished
to make sure it completes successfully.  Thanks,

Suppose there are two bitmaps [0, 256], [256, 512] and one extent
[100, 120] in the free space cache, and we want to setup a cluster
with offset=100, bytes=50.

In this case, there will be only one bitmap [256, 512] in the temporary
bitmaps list, and then setup_cluster_bitmap() won't search bitmap [0, 256].

The cause is, the list is constructed in setup_cluster_no_bitmap(),
and only bitmaps with bitmap_entry->offset >= offset will be added
into the list, and the very bitmap that convers offset has
bitmap_entry->offset <= offset.

Btrfs: fix barrier flushes

When btrfs is writing the super blocks, it send barrier flushes to make
sure writeback caching drives get all the metadata on disk in the
right order.

But, we have two bugs in the way these are sent down.  When doing
full commits (not via the tree log), we are sending the barrier down
before the last super when it should be going down before the first.

In multi-device setups, we should be waiting for the barriers to
complete on all devices before writing any of the supers.

Both of these bugs can cause corruptions on power failures.  We fix it
with some new code to send down empty barriers to all devices before
writing the first super.

Alexandre Oliva found the multi-device bug.  Arne Jansen did the async
barrier loop.

KVM guest: prevent tracing recursion with kvmclock

Prevent tracing of preempt_disable() in get_cpu_var() in
kvm_clock_read(). When CONFIG_DEBUG_PREEMPT is enabled,
preempt_disable/enable() are traced and this causes the function_graph
tracer to go into an infinite recursion. By open coding the
preempt_disable() around the get_cpu_var(), we can use the notrace
version which prevents preempt_disable/enable() from being traced and
prevents the recursion.

Based on a similar patch for Xen from Jeremy Fitzhardinge.

Newer versions of MINIX can create filesystems that allocate an extra
bitmap block.  Mounting of this succeeds, but doing a statfs call will
result in an oops in count_free because of a negative number being used
for the bh index.

Avoid this by verifying the number of allocated blocks at mount time,
erroring out if there are not enough and make statfs ignore the extras
if there are too many.

hugetlb: remove dummy definitions of HPAGE_MASK and HPAGE_SIZE

Dummy, non-zero definitions for HPAGE_MASK and HPAGE_SIZE were added in
51c6f666fceb ("mm: ZAP_BLOCK causes redundant work") to avoid a divide
by zero in generic kernel code.

That code has since been removed, but probably should never have been
added in the first place: we don't want HPAGE_SIZE to act like PAGE_SIZE
for code that is working with hugepages, for example, when the
dependency on CONFIG_HUGETLB_PAGE has not been fulfilled.

Because hugepage size can differ from architecture to architecture, each
is required to have their own definitions for both HPAGE_MASK and
HPAGE_SIZE.  This is always done in arch/*/include/asm/page.h.

So, just remove the dummy and dangerous definitions since they are no
longer needed and reveals the correct dependencies.  Tested on
architectures using the definitions with allyesconfig: x86 (even with
thp), hppa, mips, powerpc, s390, sh3, sh4, sparc, and sparc64, and with
defconfig on ia64.

This bug was introduced when moving code around to remove some forward declarations earlier, the #endif part of #ifdef CONFIG_PM_SLEEP was not moved at the same time. Now fixed by moving it to its proper place.

PM / Hibernate: Fix the early termination of test modes

Commit 2aede851ddf08666f68ffc17be446420e9d2a056
(PM / Hibernate: Freeze kernel threads after preallocating memory)
postponed the freezing of kernel threads to after preallocating memory
for hibernation. But while doing that, the hibernation test TEST_FREEZER
and the test mode HIBERNATION_TESTPROC were not moved accordingly.

As a result, when using these test modes, it only goes upto the freezing of
userspace and exits, when in fact it should go till the complete end of task
freezing stage, namely the freezing of kernel threads as well.

So, move these points of exit to appropriate places so that freezing of
kernel threads is also tested while using these test harnesses.

commit f39925dbde77 (ipv4: Cache learned redirect information in
inetpeer.) introduced a regression in ICMP redirect handling.

It assumed ipv4_dst_check() would be called because all possible routes
were attached to the inetpeer we modify in ip_rt_redirect(), but thats
not true.

commit 7cc9150ebe (route: fix ICMP redirect validation) tried to fix
this but solution was not complete. (It fixed only one route)

So we must lookup existing routes (including different TOS values) and
call check_peer_redir() on them.

I will no longer be maintaining XFS for SGI.  Ben Myers
(bpm@sgi.com) has agreed to be the primary maintainer
for XFS in my place.  I will continue to be able to push
commits to the SGI XFS tree if required.  As such I will
continue to be a designated XFS maintainer, but plan to
serve in more of a backup role.

RFC 2011 states that ICMP_MIB_INERRORS should count "the number of ICMP

ICMP-specific errors (bad ICMP checksums, bad length, etc.)."

Fix for ftdi_set_termios() glitching output

ftdi_set_termios() is constantly setting the baud rate, data bits and parity
unnecessarily on every call, . When called while characters are being
transmitted can cause the FTDI chip to corrupt the serial port bit stream
output by stalling the output half a bit during the output of a character.
Simple fix by skipping this setting if the baud rate/data bits/parity are
unchanged.

  I had a brief run with strace on the getty and it was doing ioctl()s on
  each call but it didn't look relavant to the problem. I think the issue is
  that XON/XOFF flow control was being implmented via hardware - for the ixoff
  to allow the user to use XON/XOFF to control output. Unfortunately it would
  send 3 Control URBs updating all of the settings after each piece of input

  I am trying to work around the issue of gmail messing with the tab/spacing
  by submitting via SMTP via gmail which I believe should fix the issue.

  The patch is against v3.2-rc2 and compiles - but no additional testing in
  this kernel has been done.

OHCI: final fix for NVIDIA problems (I hope)

Problems with NVIDIA's OHCI host controllers persist.  After looking
carefully through the spec, I finally realized that when a controller
is reset it then automatically goes into a SUSPEND state in which it
is completely quiescent (no DMA and no IRQs) and from which it will
not awaken until the system puts it into the OPERATIONAL state.

Therefore there's no need to worry about controllers being in the
RESET state for extended periods, or remaining in the OPERATIONAL
state during system shutdown.  The proper action for device
initialization is to put the controller into the RESET state (if it's
not there already) and then to issue a software reset.  Similarly, the
proper action for device shutdown is simply to do a software reset.

This patch (as1499) implements such an approach.  It simplifies
initialization and shutdown, and allows the NVIDIA shutdown-quirk code
to be removed.

The hardware has a restriction that the minimum ring size possible
is 128. The number of elements used is controlled by tx_pending and
the overall number of elements in the ring tx_ring_size, therefore it
is okay to limit the number of elements in use to a small value (63)
but still provide a bigger ring.

We pull one byte (the MAC header) from the first fragment before the
fragment is actually appended. So the socket buffer length is 1, not 0.

PM / shmobile: Fix build of sh7372_pm_init() for CONFIG_PM unset

Fix build regression introduced by commit 056879d2f244001b2888cdc8cf
(ARM: mach-shmobile: sh7372 A3SP no_suspend_console fix) by moving
the intialization of the A3SP domain to a separate function and
providing an empty definition of it for CONFIG_PM unset.

PM Sleep: Do not extend wakeup paths to devices with ignore_children set

Commit 4ca46ff3e0d8c234cb40ebb6457653b59584426c (PM / Sleep: Mark
devices involved in wakeup signaling during suspend) introduced
the power.wakeup_path field in struct dev_pm_info to mark devices
whose children are enabled to wake up the system from sleep states,
so that power domains containing the parents that provide their
children with wakeup power and/or relay their wakeup signals are not
turned off.  Unfortunately, that introduced a PM regression on SH7372
whose power consumption in the system "memory sleep" state increased
as a result of it, because it prevented the power domain containing
the I2C controller from being turned off when some children of that
controller were enabled to wake up the system, although the
controller was not necessary for them to signal wakeup.

To fix this issue use the observation that devices whose
power.ignore_children flag is set for runtime PM should be treated
analogously during system suspend.  Namely, they shouldn't be
included in wakeup paths going through their children.  Since the
SH7372 I2C controller's power.ignore_children flag is set, doing so
will restore the previous behavior of that SOC.

For /dev/console case, we do not kill all ldisc users. It's due to
redirected_tty_write test in __tty_hangup. In that case there still
might be a process waiting e.g. in n_tty_read for input.

We wait for such processes to disappear. The problem is that we use a
timeout. After this timeout, we continue closing the ldisc and start
freeing tty resources. It obviously leads to crashes when the other
process is woken.

So to fix this, we wait infinitely before reiniting the ldisc. (The
tiocsetd remains untouched -- times out after 5s.)

This is nicely reproducible with this run from shell:
  exec 0<>/dev/console 1<>/dev/console 2<>/dev/console
and stopping a getty like:
  systemctl stop serial-getty@ttyS0.service

The crash proper may be produced only under load or with constified
timing the same as for 92f6fa09b.

It is the only place where reinit is called from. And we really need
to wait for the old ldisc to go once. Actually this is the place where
the waiting originally was (before removed and re-added later).

This will make the fix in the following patch easier to implement.

To fix a nasty bug in ldisc hup vs. reinit we need to wait infinitely
long for ldisc to be gone. So here we add a parameter to
tty_ldisc_wait_idle to allow that.

This is only a preparation for the real fix which is done in the
following patches.
