scsi: core: Move the eh_deadline module parameter definition
The eh_deadline definition occurs in the middle of the code for
releasing a host. Avoid splitting the host release code by moving the
definition of the eh_deadline parameter to the top of the hosts.c source
file.

scsi: aacraid: Implement DropIO sync command
IOP_RESET takes a long time to complete. If controller is in a state
where we can bring it back with init struct, send a DropIO sync command
instead.
- If controller is faulted perform standard IOP_RESET in aac_srcv_init.
- If controller is not faulted get adapter properties and extended
properties.
- Update the sa_firmware variable and determine if DropIO request is
supported.
- Issue DropIO request, and get the number of outstanding commands.
- If all commands are complete with success (CT_OK), consider IOP_RESET
is complete.
- If any commands timeout, Perform the IOP_RESET.

the size of the source buffer rather than the destination:

scsi: scsi_debug: Add two new parameters to scsi_debug driver
This patch adds two new parameters to the scsi_debug driver.
During various fault injection scenarios it would be useful to be able
to pick a specific starting sector and number of follow on sectors where
a MEDIUM ERROR for reads would be returned against a scsi-debug device.
Right now this only works against sector 0x1234 and OPT_MEDIUM_ERR_NUM
follow on sectors. However during testing of md-raid and other
scenarios I wanted more flexibility.
The idea is add 2 new parameters:
medium_error_start
medium_error_count
If medium_error_start is set then we don't use the default of
OPT_MEDIUM_ERR_ADDR, but use that set value.
If medium_error_count is set we use that value otherwise default to
OPT_MEDIUM_ERR_NUM.

scsi: scsi_debug: Simplify request tag decoding
Since commit
64d513ac31bd ("scsi: use host wide tags by default") all
SCSI requests have a tag, whether or not scsi-mq is enabled.
Additionally, it is safe to use blk_mq_unique_tag() and
blk_mq_unique_tag_to_hwq() for legacy SCSI queues. Since this means that
the sdebug_mq_active variable is superfluous, remove it.

scsi: lpfc: Fix nonrecovery of NVME controller after cable swap.
In a test that is doing large numbers of cable swaps on the target, the
nvme controllers wouldn't reconnect.
During the cable swaps, the targets n_port_id would change. This
information was passed to the nvme-fc transport, in the new remoteport
registration. However, the nvme-fc transport didn't update the n_port_id
value in the remoteport struct when it reused an existing structure.
Later, when a new association was attempted on the remoteport, the
driver's NVME LS routine would use the stale n_port_id from the
remoteport struct to address the LS. As the device is no longer at that
address, the LS would go into never never land.
Separately, the nvme-fc transport will be corrected to update the
n_port_id value on a re-registration.
However, for now, there's no reason to use the transports values. The
private pointer points to the drivers node structure and the node
structure is up to date. Therefore, revise the LS routine to use the
drivers data structures for the LS. Augmented the debug message for
better debugging in the future.
Also removed a duplicate if check that seems to have slipped in.

scsi: lpfc: Treat SCSI Write operation Underruns as an error
Currently, write underruns (mismatch of amount transferred vs scsi
status and its residual) detected by the adapter are not being flagged
as an error. Its expected the target controls the data transfer and
would appropriately set the RSP values. Only read underruns are treated
as errors.
Revise the SCSI error handling to treat write underruns as an error as
well.

scsi: lpfc: Fix SCSI io host reset causing kernel crash
During SCSI error handling escalation to host reset, the SCSI io
routines were moved off the txcmplq, but the individual io's ON_CMPLQ
flag wasn't cleared. Thus, a background thread saw the io and attempted
to access it as if on the txcmplq.
Clear the flag upon removal.

enter a soft lockup on the lpfc_worker thread:

The driver was simultaneously setting a combination of flags that caused
lpfc_do_work()to effectively spin between slow path work and new event
data, causing the lockup.
Ensure in the typical wq completions, that new event data flags are set
if the slow path flag is running. The slow path will eventually
reschedule the wq handling.

scsi: lpfc: Allow set of maximum outstanding SCSI cmd limit for a target
Make the attribute writeable.
Remove the ramp up to logic as its unnecessary, simply set depth. Add
debug message if depth changed, possibly reducing limit, yet our
outstanding count has yet to catch up with it.

scsi: lpfc: Fix RQ empty firmware trap
When nvme target deferred receive logic waits for exchange resources,
the corresponding receive buffer is not replenished with the hardware.
This can result in a lack of asynchronous receive buffer resources in
the hardware, resulting in a "2885 Port Status Event: ... error
1=0x52004a01 ..." message.
Correct by replenishing the buffer whenenver the deferred logic kicks
in. Update corresponding debug messages and statistics as well.

scsi: lpfc: Fix IO failure during hba reset testing with nvme io.
A stress test repeatedly resetting the adapter while performing io would
eventually report I/O failures and missing nvme namespaces.
The driver was setting the nvmefc_fcp_req->private pointer to NULL
during the IO completion routine before upcalling done(). If the
transport was also running an abort for that IO, the driver would fail
the abort with message 6140. Failing the abort is not allowed by the
nvme-fc transport, as it mandates that the io must be returned back to
the transport. As that does not happen, the transport controller delete
has an outstanding reference and can't complete teardown.
The NULL-ing of the private pointer should be done only when the io is
considered complete. It's complete when the adapter returns the exchange
with the "exchange busy" flag clear.
Move the NULL'ing of the structure to the done case. This leaves the io
contexts set while it is busy and until the subsequent XRI_ABORTED
completion which returns the exchange is received.

scsi: lpfc: Fix PRLI handling when topology type changes
The lpfc driver does not discover a target when the topology changes
from switched-fabric to direct-connect. The target rejects the PRLI from
the initiator in direct-connect as the driver is using the old S_ID from
the switched topology.
The driver was inappropriately clearing the VP bit to register the VPI,
which is what is associated with the S_ID.
Fix by leaving the VP bit set (it was set earlier) and as the VFI is
being re-registered, set the UPDT bit.

scsi: lpfc: Add WQ Full Logic for NVME Target
I/O conditions on the nvme target may have the driver submitting to a
full hardware wq. The hardware wq is a shared resource among all nvme
controllers. When the driver hit a full wq, it failed the io posting
back to the nvme-fc transport, which then escalated it into errors.
Correct by maintaining a sideband queue within the driver that is added
to when the WQ full condition is hit, and drained from as soon as new WQ
space opens up.

scsi: lpfc: Fix frequency of Release WQE CQEs
The driver controls when the hardware sends completions that communicate
consumption of elements from the WQ. This is done by setting a WQEC bit
on a WQE.
The current driver sets it on every Nth WQE posting. However, the driver
isn't clearing the bit if the WQE is reused. Thus, if the queue depth
isn't evenly divisible by N, with enough time, it can be set on every
element, creating a lot of overhead and risking CQ full conditions.
Correct by clearing the bit when not setting it on an Nth element.

- no more kernel-side definitions of POLL... - userland ones are
visible through the entire kernel (and used pretty much only for
mangle/demangle)
- same behavior as after the first series (i.e. sparc et.al. epoll(2)
working correctly).

The next patch from Al will sort out the final differences, and we
should be all done.

xtensa: fix build with KASAN
The commit
917538e212a2 ("kasan: clean up KASAN_SHADOW_SCALE_SHIFT
usage") removed KASAN_SHADOW_SCALE_SHIFT definition from
include/linux/kasan.h and added it to architecture-specific headers,
except for xtensa. This broke the xtensa build with KASAN enabled.

powerpc/pci: Fix broken INTx configuration via OF
59f47eff03a0 ("powerpc/pci: Use of_irq_parse_and_map_pci() helper")
replaced of_irq_parse_pci() + irq_create_of_mapping() with
of_irq_parse_and_map_pci(), but neglected to capture the virq
returned by irq_create_of_mapping(), so virq remained zero, which
caused INTx configuration to fail.
Save the virq value returned by of_irq_parse_and_map_pci() and correct
the virq declaration to match the of_irq_parse_and_map_pci() signature.

kconfig: remove const qualifier from sym_expand_string_value()
This function returns realloc'ed memory, so the returned pointer
must be passed to free() when done. So, 'const' qualifier is odd.
It is allowed to modify the expanded string.

sn37, containing systems QMB700 (40x200GbE InfiniBand switch), SN3700
(32x200GbE and 16x400GbE Ethernet switch) and SN3410 (6x400GbE plus
48x50GbE Ethernet switch). These are the Top of the Rack systems, equipped
with Mellanox COM-Express carrier board and switch board with Mellanox
Quantum device, which supports InfiniBand switching with 40X200G ports and
line rate of up to HDR speed or with Mellanox Spectrum-2 device, which
supports Ethernet switching with 32X200G ports line rate of up to HDR
speed.

ibmvnic: Remove skb->protocol checks in ibmvnic_xmit
Having these checks in ibmvnic_xmit causes problems with VLAN
tagging and balance-alb/tlb bonding modes. The restriction they
imposed can be removed.

Here the chunk size for INIT_ACK packet becomes too big, mostly
because of the state cookie (INIT packet has large size with
many address parameters), plus additional server parameters.
Later this chunk causes the panic in skb_put_data():

'nskb' (head skb) was previously allocated with packet->size
from u16 'chunk->chunk_hdr->length'.
As suggested by Marcelo we should check the chunk's length in
_sctp_make_chunk() before trying to allocate skb for it and
discard a chunk if its size bigger than SCTP_MAX_CHUNK_LEN.

s390/qeth: fix underestimated count of buffer elements
For a memory range/skb where the last byte falls onto a page boundary
(ie. 'end' is of the form xxx...xxx001), the PFN_UP() part of the
calculation currently doesn't round up to the next PFN due to an
off-by-one error.
Thus qeth believes that the skb occupies one page less than it
actually does, and may select a IO buffer that doesn't have enough spare
buffer elements to fit all of the skb's data.
HW detects this as a malformed buffer descriptor, and raises an
exception which then triggers device recovery.

net: stmmac: remove redundant enable of PMT irq
For dwmac4, GMAC_INT_DEFAULT_ENABLE already includes
GMAC_INT_PMT_EN, so it is redundant to check if hw->pmt
is set, and if so, setting the bit again.
For dwmac1000, GMAC_INT_DEFAULT_MASK does not include
GMAC_INT_DISABLE_PMT, so it is redundant to check if
hw->pmt is set, and if so, clearing an already cleared bit.
Improve code readability by removing this redundant code.

net: stmmac: rename GMAC_INT_DEFAULT_MASK for dwmac4
GMAC_INT_DEFAULT_MASK is written to the interrupt enable register.
In previous versions of the IP (e.g. dwmac1000), this register was
instead an interrupt mask register.

net: stmmac: discard disabled flags in interrupt status register
The interrupt status register in both dwmac1000 and dwmac4 ignores
interrupt enable (for dwmac4) / interrupt mask (for dwmac1000).
Therefore, if we want to check only the bits that can actually trigger
an irq, we have to filter the interrupt status register manually.
Commit
0a764db10337 ("stmmac: Discard masked flags in interrupt status
register") fixed this for dwmac1000. Fix the same issue for dwmac4.
Just like commit
0a764db10337 ("stmmac: Discard masked flags in
interrupt status register"), this makes sure that we do not get
spurious link up/link down prints.

ibmvnic: Reset long term map ID counter
When allocating RX or TX buffer pools, the driver needs to provide a
unique mapping ID to firmware for each pool. This value is assigned
using a counter which is incremented after a new pool is created. The
ID can be an integer ranging from 1-255. When migrating to a device
that requests a different number of queues, this value was not being
reset properly. As a result, after enough migrations, the counter
exceeded the upper bound and pool creation failed. This is fixed by
resetting the counter to one in this case.

installing empty man pages; the man page for bpftool-map is corrected
and a set of missing bash completions added in order to avoid shipping
bpftool where the completions are only partially working, from Quentin.
4) Fix applying the relocation to immediate load instructions in the
nfp JIT which were missing a shift, from Jakub.
5) Two fixes for the BPF kernel selftests: handle CONFIG_BPF_JIT_ALWAYS_ON=y
gracefully in test_bpf.ko module and mark them as FLAG_EXPECTED_FAIL
in this case; and explicitly delete the veth devices in the two tests
test_xdp_{meta,redirect}.sh before dismantling the netnses as when
selftests are run in batch mode, then workqueue to handle destruction
might not have finished yet and thus veth creation in next test under
same dev name would fail, from Yonghong.
6) Fix test_kmod.sh to check the test_bpf.ko module path before performing
an insmod, and fallback to modprobe. Especially the latter is useful
when having a device under test that has the modules installed instead,
from Naresh.

vulnerabilities. But there are other updates as well, the changes in

- An s390 specific implementation of the array_index_mask_nospec
function to the defense against spectre v1
- Two patches to utilize the new PPA-12/PPA-13 instructions to run
the kernel and/or user space with reduced branch predicton.
- The s390 variant of the 'retpoline' spectre v2 defense called
'expoline'. There is no return instruction for s390, instead an
indirect branch is used for function return
The s390 defense mechanism for indirect branches works by using an
execute-type instruction with the indirect branch as the target of
the execute. In effect that turns off the prediction for the
indirect branch.
- Scrub registers in entry.S that contain user controlled values to
prevent the speculative use of these values.
- Re-add the second parameter for the s390 specific runtime
instrumentation system call and move the header file to uapi. The
second parameter will continue to do nothing but older kernel
versions only accepted valid real-time signal numbers. The details

KVM: PPC: Book3S HV: Branch inside feature section
We ended up with code that did a conditional branch inside a feature
section to code outside of the feature section. Depending on how the
object file gets organized, that might mean we exceed the 14bit
relocation limit for conditional branches:
arch/powerpc/kvm/built-in.o:arch/powerpc/kvm/book3s_hv_rmhandlers.S:416:(__ftr_alt_97+0x8): relocation truncated to fit: R_PPC64_REL14 against `.text'+1ca4
So instead of doing a conditional branch outside of the feature section,
let's just jump at the end of the same, making the branch very short.

KVM: PPC: Book3S HV: Make HPT resizing work on POWER9
This adds code to enable the HPT resizing code to work on POWER9,
which uses a slightly modified HPT entry format compared to POWER8.
On POWER9, we convert HPTEs read from the HPT from the new format to
the old format so that the rest of the HPT resizing code can work as
before. HPTEs written to the new HPT are converted to the new format
as the last step before writing them into the new HPT.
This takes out the checks added by commit
bcd3bb63dbc8 ("KVM: PPC:
Book3S HV: Disable HPT resizing on POWER9 for now", 2017-02-18),
now that HPT resizing works on POWER9.
On POWER9, when we pivot to the new HPT, we now call
kvmppc_setup_partition_table() to update the partition table in order
to make the hardware use the new HPT.

KVM: PPC: Book3S HV: Fix handling of secondary HPTEG in HPT resizing code
This fixes the computation of the HPTE index to use when the HPT
resizing code encounters a bolted HPTE which is stored in its
secondary HPTE group. The code inverts the HPTE group number, which
is correct, but doesn't then mask it with new_hash_mask. As a result,
new_pteg will be effectively negative, resulting in new_hptep
pointing before the new HPT, which will corrupt memory.
In addition, this removes two BUG_ON statements. The condition that
the BUG_ONs were testing -- that we have computed the hash value
incorrectly -- has never been observed in testing, and if it did
occur, would only affect the guest, not the host. Given that
BUG_ON should only be used in conditions where the kernel (i.e.
the host kernel, in this case) can't possibly continue execution,
it is not appropriate here.
