sched: Push put_prev_task() into pick_next_task()
In order to avoid having to do put/set on a whole cgroup hierarchy
when we context switch, push the put into pick_next_task() so that
both operations are in the same function. Further changes then allow
us to possibly optimize away redundant work.

sched/fair: Track cgroup depth
Track depth in cgroup tree, this is useful for things like
find_matching_se() where you need to get to a common parent of two
sched entities.
Keeping the depth avoids having to calculate it on the spot, which
saves a number of possible cache-misses.

sched: Move rq->idle_stamp up to the core
idle_balance() modifies the rq->idle_stamp field, making this information
shared across core.c and fair.c.
As we know if the cpu is going to idle or not with the previous patch, let's
encapsulate the rq->idle_stamp information in core.c by moving it up to the
caller.
The idle_balance() function returns true in case a balancing occured and the
cpu won't be idle, false if no balance happened and the cpu is going idle.

sched: Fix race in idle_balance()
The scheduler main function 'schedule()' checks if there are no more tasks
on the runqueue. Then it checks if a task should be pulled in the current
runqueue in idle_balance() assuming it will go to idle otherwise.
But idle_balance() releases the rq->lock in order to look up the sched
domains and takes the lock again right after. That opens a window where
another cpu may put a task in our runqueue, so we won't go to idle but
we have filled the idle_stamp, thinking we will.
This patch closes the window by checking if the runqueue has been modified
but without pulling a task after taking the lock again, so we won't go to idle
right after in the __schedule() function.

sched: Implement task_nice() as static inline function
As patch "sched: Move the priority specific bits into a new header file" exposes
the priority related macros in linux/sched/prio.h, we don't have to implement
task_nice() in kernel/sched/core.c any more.
This patch implements it in linux/sched/sched.h as static inline function,
saving the kernel stack and enhancing performance a bit.

sched: Expose some macros related to priority
Some macros in kernel/sched/sched.h about priority are
private to kernel/sched. But they are useful to other
parts of the core kernel.
This patch moves these macros from kernel/sched/sched.h to

sched/deadline: Skip in switched_to_dl() if task is current
When p is current and it's not of dl class, then there are no other
dl taks in the rq. If we had had pushable tasks in some other rq,
they would have been pushed earlier. So, skip "p == rq->curr" case.

afs: proc cells and rootcell are writeable
Both proc files are writeable and used for configuring cells. But
there is missing correct mode flag for writeable files. Without
this patch both proc files are read only.
[ It turns out they aren't really read-only, since root can write to
them even if the write bit isn't set due to CAP_DAC_OVERRIDE ]

tile: remove compat_sys_lookup_dcookie declaration to fix compile error
With commit
d8d14bd09cdd ("fs/compat: fix lookup_dcookie() parameter
handling") I changed the type of the len parameter of the
lookup_dcookie() syscall.
However I missed that there was still a stale declaration in
arch/tile/.. which now causes a compile error on tile:

Revert "PCI: Remove from bus_list and release resources in pci_release_dev()"
Revert commit
ef83b0781a73 "PCI: Remove from bus_list and release
resources in pci_release_dev()" that made some nasty race conditions
become possible. For example, if a Thunderbolt link is unplugged
and then replugged immediately, the pci_release_dev() resulting from
the hot-remove code path may be racing with the hot-add code path
which after that commit causes various kinds of breakage to happen
(up to and including a hard crash of the whole system).
Moreover, the problem that commit
ef83b0781a73 attempted to address
cannot happen any more after commit
8a4c5c329de7 "PCI: Check parent
kobject in pci_destroy_dev()", because pci_destroy_dev() will now
return immediately if it has already been executed for the given
device.
Note, however, that the invocation of msi_remove_pci_irq_vectors()
removed by commit
ef83b0781a73 from pci_free_resources() along with
the other changes made by it is not added back because of subsequent
code changes depending on that modification.

Fix mountpoint reference leakage in linkat
Recent changes to retry on ESTALE in linkat
(commit
442e31ca5a49e398351b2954b51f578353fdf210)
introduced a mountpoint reference leak and a small memory
leak in case a filesystem link operation returns ESTALE
which is pretty normal for distributed filesystems like
lustre, nfs and so on.
Free old_path in such a case.

hfsplus: use xattr handlers for removexattr
hfsplus was already using the handlers for get and set operations,
and with the removal of can_set_xattr we've now allow operations that
wouldn't otherwise be allowed.
With this we can also centralize the special-casing of the osx.
attrs that don't have prefixes on disk in the osx xattr handlers.

fs/super.c: sync ro remount after blocking writers
Move sync_filesystem() after sb_prepare_remount_readonly(). If writers
sneak in anywhere from sync_filesystem() to sb_prepare_remount_readonly()
it can cause inodes to be dirtied and writeback to occur well after
sys_mount() has completely successfully.
This was spotted by corrupted ubifs filesystems on reboot, but appears
that it can cause issues with any filesystem using writeback.

- Recent cpufreq changes related to the "boost" (turbo) feature broke
the acpi-cpufreq error code path causing a NULL pointer dereference
to occur on some systems. Fix from Konrad Rzeszutek Wilk.
- The log level of a CPU initialization error message added recently
needs to be reduced, because the particular BIOS issue indicated by
it turns out to be widespread and doesn't really matter for the
majority of systems having it. From Jiang Liu.
- The regulator API needs to be told to stay away from things on systems
with ACPI BIOSes or it may conflict with the BIOS' own handling of
voltage regulators. Fix from Mark Brown that works around a 3.13
regression in lm90 on PCs occuring if the regulator API is enabled.
- Prevent the Exynos4 devfreq driver from being built on multiplatform,
because it depends on things that aren't available during such builds.

- Upstream ACPICA doesn't use the bool type as defined in the kernel,
so modify the kernel's ACPICA code to follow the upstream in that
respect (only one variable definition is affected) to reduce

- Make the ACPI device PM code use ACPI_COMPANION() instead of its own
routine doing the same thing (and invokng ACPI_COMPANION() in the
process).
- Modify some routines in the ACPI processor driver to follow the

alpha: fix broken network checksum
The patch
3ddc5b46a8e90f3c9251338b60191d0a804b0d92 breaks networking on
alpha (there is a follow-up fix
5cfe8f1ba5eebe6f4b6e5858cdb1a5be4f3272a6,
but networking is still broken even with the second patch).
The patch
3ddc5b46a8e90f3c9251338b60191d0a804b0d92 makes
csum_partial_copy_from_user check the pointer with access_ok. However,
csum_partial_copy_from_user is called also from csum_partial_copy_nocheck
and csum_partial_copy_nocheck is called on kernel pointers and it is
supposed not to check pointer validity.
This bug results in ssh session hangs if the system is loaded and bulk
data are printed to ssh terminal.
This patch fixes csum_partial_copy_nocheck to call set_fs(KERNEL_DS), so
that access_ok in csum_partial_copy_from_user accepts kernel-space
addresses.

ALSA: hda/hdmi - allow PIN_OUT to be dynamically enabled
Commit
384a48d71520 "ALSA: hda: HDMI: Support codecs with fewer cvts
than pins" dynamically enabled each pin widget's PIN_OUT only when the
pin was actively in use. This was required on certain NVIDIA CODECs for
correct operation. Specifically, if multiple pin widgets each had their
mux input select the same audio converter widget and each pin widget had
PIN_OUT enabled, then only one of the pin widgets would actually receive
the audio, and often not the one the user wanted!
However, this apparently broke some Intel systems, and commit
6169b673618b "ALSA: hda - Always turn on pins for HDMI/DP" reverted the
dynamic setting of PIN_OUT. This in turn broke the afore-mentioned NVIDIA
CODECs.
This change supports either dynamic or static handling of PIN_OUT,
selected by a flag set up during CODEC initialization. This flag is
enabled for all recent NVIDIA GPUs.

cifs: Fix check for regular file in couldbe_mf_symlink()
MF Symlinks are regular files containing content in a specified format.
The function couldbe_mf_symlink() checks the mode for a set S_IFREG bit
as a test to confirm that it is a regular file. This bit is also set for
other filetypes and simply checking for this bit being set may return
false positives.
We ensure that we are actually checking for a regular file by using the
S_ISREG macro to test instead.

xen/pvh: Fix misplaced kfree from xlated_setup_gnttab_pages
Passing a freed 'pages' to free_xenballooned_pages will end badly
on kernels with slub debug enabled.
This looks out of place between the rc assign and the check, but
we do want to kfree pages regardless of which path we take.

drivers: xen: deaggressive selfballoon driver
Current xen-selfballoon driver is too aggressive which may cause OOM be
triggered more often. Eg. this bug reported by James:

There are two mainly reasons:
1) The original goal_page didn't consider some pages used by kernel space, like
slab pages and pages used by device drivers.
2) The balloon driver may not give back memory to guest OS fast enough when the
workload suddenly aquries a lot of physical memory.
In both cases, the guest OS will suffer from memory pressure and OOM may
be triggered.
The fix is make xen-selfballoon driver not that aggressive by adding extra 10%
of total ram pages to goal_page.
It's more valuable to keep the guest system reliable and response faster than
balloon out these 10% pages to XEN.

xen/grant-table: Avoid m2p_override during mapping
The grant mapping API does m2p_override unnecessarily: only gntdev needs it,
for blkback and future netback patches it just cause a lock contention, as
those pages never go to userspace. Therefore this series does the following:
- the original functions were renamed to __gnttab_[un]map_refs, with a new
parameter m2p_override
- based on m2p_override either they follow the original behaviour, or just set
the private flag and call set_phys_to_machine
- gnttab_[un]map_refs are now a wrapper to call __gnttab_[un]map_refs with
m2p_override false
- a new function gnttab_[un]map_refs_userspace provides the old behaviour
It also removes a stray space from page.h and change ret to 0 if
XENFEAT_auto_translated_physmap, as that is the only possible return value
there.

arm64: mm: Introduce PTE_WRITE
We have the following means for encoding writable or dirty ptes:
PTE_DIRTY PTE_RDONLY
!pte_dirty && !pte_write 0 1
!pte_dirty && pte_write 0 1
pte_dirty && !pte_write 1 1
pte_dirty && pte_write 1 0
So we can't distinguish between writable clean ptes and read only
ptes. This can cause problems with ptes being incorrectly flagged as
read only when they are writable but not dirty.
This patch introduces a new software bit PTE_WRITE which allows us to
correctly identify writable ptes. PTE_RDONLY is now only clear for
valid ptes where a page is both writable and dirty.

The timeout pointer parameter is provided by userland (hence the __user
annotation) but for x32 syscalls it's simply cast to a kernel pointer
and is passed to __sys_recvmmsg which will eventually directly
dereference it for both reading and writing. Other callers to
__sys_recvmmsg properly copy from userland to the kernel first.
The bug was introduced by commit
ee4fa23c4bfc ("compat: Use
COMPAT_USE_64BIT_TIME in net/compat.c") and should affect all kernels
since 3.4 (and perhaps vendor kernels if they backported x32 support
along with this code).
Note that CONFIG_X86_X32_ABI gets enabled at build time and only if
CONFIG_X86_X32 is enabled and ld can build x32 executables.
Other uses of COMPAT_USE_64BIT_TIME seem fine.
This addresses CVE-2014-0038.

memcg: fix mutex not unlocked on memcg_create_kmem_cache fail path
Commit
842e2873697e ("memcg: get rid of kmem_cache_dup()") introduced a
mutex for memcg_create_kmem_cache() to protect the tmp_name buffer that
holds the memcg name. It failed to unlock the mutex if this buffer
could not be allocated.
This patch fixes the issue by appropriately unlocking the mutex if the
allocation fails.

mm, oom: base root bonus on current usage
A 3% of system memory bonus is sometimes too excessive in comparison to
other processes.
With commit
a63d83f427fb ("oom: badness heuristic rewrite"), the OOM
killer tries to avoid killing privileged tasks by subtracting 3% of
overall memory (system or cgroup) from their per-task consumption. But
as a result, all root tasks that consume less than 3% of overall memory
are considered equal, and so it only takes 33+ privileged tasks pushing
the system out of memory for the OOM killer to do something stupid and
kill dhclient or other root-owned processes. For example, on a 32G
machine it can't tell the difference between the 1M agetty and the 10G
fork bomb member.
The changelog describes this 3% boost as the equivalent to the global
overcommit limit being 3% higher for privileged tasks, but this is not
the same as discounting 3% of overall memory from _every privileged task
individually_ during OOM selection.
Replace the 3% of system memory bonus with a 3% of current memory usage
bonus.
By giving root tasks a bonus that is proportional to their actual size,
they remain comparable even when relatively small. In the example
above, the OOM killer will discount the 1M agetty's 256 badness points
down to 179, and the 10G fork bomb's 262144 points down to 183500 points
and make the right choice, instead of discounting both to 0 and killing
agetty because it's first in the task list.

mm: don't lose the SOFT_DIRTY flag on mprotect
The SOFT_DIRTY bit shows that the content of memory was changed after a
defined point in the past. mprotect() doesn't change the content of
memory, so it must not change the SOFT_DIRTY bit.
This bug causes a malfunction: on the first iteration all pages are
dumped. On other iterations only pages with the SOFT_DIRTY bit are
dumped. So if the SOFT_DIRTY bit is cleared from a page by mistake, the
page is not dumped and its content will be restored incorrectly.
This patch does nothing with _PAGE_SWP_SOFT_DIRTY, becase pte_modify()
is called only for present pages.

mm/slub.c: fix page->_count corruption (again)
Commit
abca7c496584 ("mm: fix slab->page _count corruption when using
slub") notes that we can not _set_ a page->counters directly, except
when using a real double-cmpxchg. Doing so can lose updates to
->_count.
That is an absolute rule:
You may not *set* page->counters except via a cmpxchg.
Commit
abca7c496584 fixed this for the folks who have the slub
cmpxchg_double code turned off at compile time, but it left the bad case
alone. It can still be reached, and the same bug triggered in two
cases:
1. Turning on slub debugging at runtime, which is available on
the distro kernels that I looked at.
2. On 64-bit CPUs with no CMPXCHG16B (some early AMD x86-64
cpus, evidently)
There are at least 3 ways we could fix this:
1. Take all of the exising calls to cmpxchg_double_slab() and
__cmpxchg_double_slab() and convert them to take an old, new
and target 'struct page'.
2. Do (1), but with the newly-introduced 'slub_data'.
3. Do some magic inside the two cmpxchg...slab() functions to
pull the counters out of new_counters and only set those
fields in page->{inuse,frozen,objects}.
I've done (2) as well, but it's a bunch more code. This patch is an
attempt at (3). This was the most straightforward and foolproof way
that I could think to do this.
This would also technically allow us to get rid of the ugly
#if defined(CONFIG_HAVE_CMPXCHG_DOUBLE) && \
defined(CONFIG_HAVE_ALIGNED_STRUCT_PAGE)
in 'struct page', but leaving it alone has the added benefit that
'counters' stays 'unsigned' instead of 'unsigned long', so all the
copies that the slub code does stay a bit smaller.

mm/mempolicy.c: fix mempolicy printing in numa_maps
As a result of commit
5606e3877ad8 ("mm: numa: Migrate on reference
policy"), /proc/<pid>/numa_maps prints the mempolicy for any <pid> as
"prefer:N" for the local node, N, of the process reading the file.
This should only be printed when the mempolicy of <pid> is
MPOL_PREFERRED for node N.
If the process is actually only using the default mempolicy for local
node allocation, make sure "default" is printed as expected.

zram: remove zram->lock in read path and change it with mutex
Finally, we separated zram->lock dependency from 32bit stat/ table
handling so there is no reason to use rw_semaphore between read and
write path so this patch removes the lock from read path totally and
changes rw_semaphore with mutex. So, we could do

zram: remove workqueue for freeing removed pending slot
Commit
a0c516cbfc74 ("zram: don't grab mutex in zram_slot_free_noity")
introduced free request pending code to avoid scheduling by mutex under
spinlock and it was a mess which made code lenghty and increased
overhead.
Now, we don't need zram->lock any more to free slot so this patch
reverts it and then, tb_lock should protect it.

zram: introduce zram->tb_lock
Currently, the zram table is protected by zram->lock but it's rather
coarse-grained lock and it makes hard for scalibility.
Let's use own rwlock instead of depending on zram->lock. This patch
adds new locking so obviously, it would make slow but this patch is just
prepartion for removing coarse-grained rw_semaphore(ie, zram->lock)
which is hurdle about zram scalability.
Final patch in this patchset series will remove the lock from read-path
and change rw_semaphore with mutex in write path. With bonus, we could
drop pending slot free mess in next patch.

zram: use atomic operation for stat
Some of fields in zram->stats are protected by zram->lock which is
rather coarse-grained so let's use atomic operation without explict
locking.
This patch is ready for removing dependency of zram->lock in read path
which is very coarse-grained rw_semaphore. Of course, this patch adds
new atomic operation so it might make slow but my 12CPU test couldn't
spot any regression. All gain/lose is marginal within stddev.

zram: remove unnecessary free
Commit
a0c516cbfc74 ("zram: don't grab mutex in zram_slot_free_noity")
introduced pending zram slot free in zram's write path in case of
missing slot free by memory allocation failure in zram_slot_free_notify
but it is not necessary because we have already freed the slot right
before overwriting.

zram: delay pending free request in read path
Sergey reported we don't need to handle pending free request every I/O
so that this patch removes it in read path while we remain it in write
path.
Let's consider below example.
Swap subsystem ask to zram "A" block free by swap_slot_free_notify but
zram had been pended it without real freeing. Swap subsystem allocates
"A" block for new data but request pended for a long time just handled
and zram blindly free new data on the "A" block. :(
That's why we couldn't remove handle pending free request right before
zram-write.

zram: fix race between reset and flushing pending work
Dan and Sergey reported that there is a racy between reset and flushing
of pending work so that it could make oops by freeing zram->meta in
reset while zram_slot_free can access zram->meta if new request is
adding during the race window.
This patch moves flush after taking init_lock so it prevents new request
so that it closes the race.

zram: promote zram from staging
Zram has lived in staging for a LONG LONG time and have been
fixed/improved by many contributors so code is clean and stable now. Of
course, there are lots of product using zram in real practice.
The major TV companys have used zram as swap since two years ago and
recently our production team released android smart phone with zram
which is used as swap, too and recently Android Kitkat start to use zram
for small memory smart phone. And there was a report Google released
their ChromeOS with zram, too and cyanogenmod have been used zram long
time ago. And I heard some disto have used zram block device for tmpfs.
In addition, I saw many report from many other peoples. For example,
Lubuntu start to use it.
The benefit of zram is very clear. With my experience, one of the
benefit was to remove jitter of video application with backgroud memory
pressure. It would be effect of efficient memory usage by compression
but more issue is whether swap is there or not in the system. Recent
mobile platforms have used JAVA so there are many anonymous pages. But
embedded system normally are reluctant to use eMMC or SDCard as swap
because there is wear-leveling and latency issues so if we do not use
swap, it means we can't reclaim anoymous pages and at last, we could
encounter OOM kill. :(
Although we have real storage as swap, it was a problem, too. Because
it sometime ends up making system very unresponsible caused by slow swap
storage performance.

Other uses case is to use zram for block device. Zram is block device
so anyone can format the block device and mount on it so some guys on
the internet start zram as /var/tmp.

zsmalloc: move it under mm
This patch moves zsmalloc under mm directory.
Before that, description will explain why we have needed custom
allocator.
Zsmalloc is a new slab-based memory allocator for storing compressed
pages. It is designed for low fragmentation and high allocation success
rate on large object, but <= PAGE_SIZE allocations.
zsmalloc differs from the kernel slab allocator in two primary ways to
achieve these design goals.
zsmalloc never requires high order page allocations to back slabs, or
"size classes" in zsmalloc terms. Instead it allows multiple
single-order pages to be stitched together into a "zspage" which backs
the slab. This allows for higher allocation success rate under memory
pressure.
Also, zsmalloc allows objects to span page boundaries within the zspage.
This allows for lower fragmentation than could be had with the kernel
slab allocator for objects between PAGE_SIZE/2 and PAGE_SIZE. With the
kernel slab allocator, if a page compresses to 60% of it original size,
the memory savings gained through compression is lost in fragmentation
because another object of the same size can't be stored in the leftover
space.
This ability to span pages results in zsmalloc allocations not being
directly addressable by the user. The user is given an
non-dereferencable handle in response to an allocation request. That
handle must be mapped, using zs_map_object(), which returns a pointer to
the mapped region that can be used. The mapping is necessary since the
object data may reside in two different noncontigious pages.
The zsmalloc fulfills the allocation needs for zram perfectly

kernel/smp.c: remove cpumask_ipi
After commit
9a46ad6d6df3 ("smp: make smp_call_function_many() use logic
similar to smp_call_function_single()"), cfd->cpumask is accessed only
in smp_call_function_many(). So there is no more need to copy it into
cfd->cpumask_ipi before putting csd into the list. The cpumask_ipi
field is obsolete and can be removed.

drivers/video/backlight/lcd.c: call put_device if device_register fails
Currently we kfree the container of the device which failed to register.
This is wrong as the last reference is not given up with a put_device
call. Also, now that we have put_device() callen, we no longer need the
kfree as the new_ld->dev.release function will take care of kfreeing the
associated memory.

nfs: fix xattr inode op pointers when disabled
Chris Mason reported a NULL pointer derefernence in generic_getxattr()
that was due to sb->s_xattr being NULL.
The reason is that the nfs #ifdef's for ACL support were misplaced, and
the nfs3 inode operations had the xattr operation pointers set up, even
though xattrs were not actually supported. As a result, the xattr code
was being called without the infrastructure having been set up.
Move the #ifdef's appropriately.

x86, build: Build 16-bit code with -m16 where possible
Both clang 3.5 and GCC 4.9 will support this (as of r199754 and r207196
respectively). Both have been tested to produce booting kernels when the
16-bit code is built with -m16. (Modulo LLVM PR3997, at least.)

x86, boot: Fix word-size assumptions in has_eflag() inline asm
Commit
dd78b97367bd575918204cc89107c1479d3fc1a7 ("x86, boot: Move CPU
flags out of cpucheck") introduced ambiguous inline asm in the
has_eflag() function. In 16-bit mode want the instruction to be
'pushfl', but we just say 'pushf' and hope the compiler does what we
wanted.
When building with 'clang -m16', it won't, because clang doesn't use
the horrid '.code16gcc' hack that even 'gcc -m16' uses internally.
Say what we mean and don't make the compiler make assumptions.
[ hpa: ideally we would be able to use the gcc %zN construct here, but
that is broken for 64-bit integers in gcc < 4.5.
The code with plain "pushf/popf" is fine for 32- or 64-bit mode, but
not for 16-bit mode; in 16-bit mode those are 16-bit instructions in
.code16 mode, and 32-bit instructions in .code16gcc mode. ]

arm64: FIQs are unused
So any FIQ handling is superfluous at the moment. The functions to
disable/enable FIQs is kept around if ever someone needs them in the
future, but existing calling sites including arch_cpu_idle_prepare()
may go for now.

xen/gnttab: Use phys_addr_t to describe the grant frame base address
On ARM, address size can be 32 bits or 64 bits (if CONFIG_ARCH_PHYS_ADDR_T_64BIT
is enabled).
We can't assume that the grant frame base address will always fits in an
unsigned long. Use phys_addr_t instead of unsigned long as argument for
gnttab_setup_auto_xlat_frames.

xen: swiotlb: handle sizeof(dma_addr_t) != sizeof(phys_addr_t)
The use of phys_to_machine and machine_to_phys in the phys<=>bus conversions
causes us to lose the top bits of the DMA address if the size of a DMA address is not the same as the size of the phyiscal address.
This can happen in practice on ARM where foreign pages can be above 4GB even
though the local kernel does not have LPAE page tables enabled (which is
totally reasonable if the guest does not itself have >4GB of RAM). In this
case the kernel still maps the foreign pages at a phys addr below 4G (as it
must) but the resulting DMA address (returned by the grant map operation) is
much higher.
This is analogous to a hardware device which has its view of RAM mapped up
high for some reason.
This patch makes I/O to foreign pages (specifically blkif) work on 32-bit ARM
systems with more than 4GB of RAM.

arm/xen: Initialize event channels earlier
Event channels driver needs to be initialized very early. Until now, Xen
initialization was done after all CPUs was bring up.
We can safely move the initialization to an early initcall.
Also use a cpu notifier to:
- Register the VCPU when the CPU is prepared
- Enable event channel IRQ when the CPU is running

target: Fix percpu_ref_put race in transport_lun_remove_cmd
This patch fixes a percpu_ref_put race for se_lun->lun_ref in
transport_lun_remove_cmd() where ->lun_ref could end up being
put more than once per command via different target completion
and fabric release contexts.
It adds a cmpxchg() for se_cmd->lun_ref_active to ensure that
percpu_ref_put() is only ever called once per se_cmd.
This bug was manifesting itself as a LUN shutdown regression
bug in >= v3.13 code, where percpu_ref_kill() would end up
hanging indefinately due to the incorrect percpu_ref count.
(Change se_cmd->lun_ref_active from bool -> int to force at
least a 4-byte cmpxchg with MIPS ll/sc ins. - Fengguang)

target/iscsi: Fix network portal creation race
When creating network portals rapidly, such as when restoring a
configuration, LIO's code to reuse existing portals can return a false
negative if the thread hasn't run yet and set np_thread_state to
ISCSI_NP_THREAD_ACTIVE. This causes an error in the network stack
when attempting to bind to the same address/port.
This patch sets NP_THREAD_ACTIVE before the np is placed on g_np_list,
so even if the thread hasn't run yet, iscsit_get_np will return the
existing np.
Also, convert np_lock -> np_mutex + hold across adding new net portal
to g_np_list to prevent a race where two threads may attempt to create
the same network portal, resulting in one of them failing.
