KVM: PPC: Book3S: PR: Make svcpu -> vcpu store preempt savvy

As soon as we get back to our "highmem" handler in virtual address
space we may get preempted. Today the reason we can get preempted is
that we replay interrupts and all the lazy logic thinks we have
interrupts enabled.

However, it's not hard to make the code interruptible and that way
we can enable and handle interrupts even earlier.

This fixes random guest crashes that happened with CONFIG_PREEMPT=y
for me.

The kvmppc_copy_{to,from}_svcpu functions are publically visible,
so we should also export them in a header for others C files to
consume.

So far we didn't need this because we only called it from asm code.
The next patch will introduce a C caller.

We call a C helper to save all svcpu fields into our vcpu. The C
ABI states that r12 is considered volatile. However, we keep our
exit handler id in r12 currently.

So we need to save it away into a non-volatile register instead
that definitely does get preserved across the C call.

This bug usually didn't hit anyone yet since gcc is smart enough
to generate code that doesn't even need r12 which means it stayed
identical throughout the call by sheer luck. But we can't rely on
that.

Since kvmppc_hv_find_lock_hpte() is called from both virtmode and
realmode, so it can trigger the deadlock.

Suppose the following scene:

Two physical cpuM, cpuN, two VM instances A, B, each VM has a group of
vcpus.

If on cpuM, vcpu_A_1 holds bitlock X (HPTE_V_HVLOCK), then is switched
out, and on cpuN, vcpu_A_2 try to lock X in realmode, then cpuN will be
caught in realmode for a long time.

What makes things even worse if the following happens,
  On cpuM, bitlockX is hold, on cpuN, Y is hold.
  vcpu_B_2 try to lock Y on cpuM in realmode
  vcpu_A_2 try to lock X on cpuN in realmode

Oops! deadlock happens

Some users have reported instances of the host hanging with secondary
threads of a core waiting for the primary thread to exit the guest,
and the primary thread stuck in nap mode.  This prompted a review of
the memory barriers in the guest entry/exit code, and this is the
result.  Most of these changes are the suggestions of Dean Burdick
<deanburdick@us.ibm.com>.

The barriers between updating napping_threads and reading the
entry_exit_count on the one hand, and updating entry_exit_count and
reading napping_threads on the other, need to be isync not lwsync,
since we need to ensure that either the napping_threads update or the
entry_exit_count update get seen.  It is not sufficient to order the
load vs. lwarx, as lwsync does; we need to order the load vs. the
stwcx., so we need isync.

In addition, we need a full sync before sending IPIs to wake other
threads from nap, to ensure that the write to the entry_exit_count is
visible before the IPI occurs.

KVM: PPC: Book3S HV: Fix physical address calculations

This fixes a bug in kvmppc_do_h_enter() where the physical address
for a page can be calculated incorrectly if transparent huge pages
(THP) are active.  Until THP came along, it was true that if we
encountered a large (16M) page in kvmppc_do_h_enter(), then the
associated memslot must be 16M aligned for both its guest physical
address and the userspace address, and the physical address
calculations in kvmppc_do_h_enter() assumed that.  With THP, that
is no longer true.

In the case where we are using MMU notifiers and the page size that
we get from the Linux page tables is larger than the page being mapped
by the guest, we need to fill in some low-order bits of the physical
address.  Without THP, these bits would be the same in the guest
physical address (gpa) and the host virtual address (hva).  With THP,
they can be different, and we need to use the bits from hva rather
than gpa.

In the case where we are not using MMU notifiers, the host physical
address we get from the memslot->arch.slot_phys[] array already
includes the low-order bits down to the PAGE_SIZE level, even if
we are using large pages.  Thus we can simplify the calculation in
this case to just add in the remaining bits in the case where
PAGE_SIZE is 64k and the guest is mapping a 4k page.

The same bug exists in kvmppc_book3s_hv_page_fault().  The basic fix
is to use psize (the page size from the HPTE) rather than pte_size
(the page size from the Linux PTE) when updating the HPTE low word
in r.  That means that pfn needs to be computed to PAGE_SIZE
granularity even if the Linux PTE is a huge page PTE.  That can be
arranged simply by doing the page_to_pfn() before setting page to
the head of the compound page.  If psize is less than PAGE_SIZE,
then we need to make sure we only update the bits from PAGE_SIZE
upwards, in order not to lose any sub-page offset bits in r.
On the other hand, if psize is greater than PAGE_SIZE, we need to
make sure we don't bring in non-zero low order bits in pfn, hence
we mask (pfn << PAGE_SHIFT) with ~(psize - 1).

ERROR: "__divdi3" [fs/hfsplus/hfsplus.ko] undefined!

Introduced by commit 099e9245e04d ("hfsplus: implement attributes file's
header node initialization code").

i_size_read() returns loff_t, which is long long, i.e.  64-bit.  node_size
is size_t, which is either 32-bit or 64-bit.  Hence
"i_size_read(attr_file) / node_size" is a 64-by-32 or 64-by-64 division,
causing (some versions of) gcc to emit a call to __divdi3().

Fortunately node_size is actually 16-bit, as the sole caller of
hfsplus_init_header_node() passes a u16.  Hence change its type from
size_t to u16, and use do_div() to perform a 64-by-32 division.

Not seen in m68k/allmodconfig in -next, so it really depends on the
verion of gcc.

kfifo API type safety

This patch enhances the type safety for the kfifo API.  It is now safe
to put const data into a non const FIFO and the API will now generate a
compiler warning when reading from the fifo where the destination
address is pointing to a const variable.

As a side effect the kfifo_put() does now expect the value of an element
instead a pointer to the element.  This was suggested Russell King.  It
make the handling of the kfifo_put easier since there is no need to
create a helper variable for getting the address of a pointer or to pass
integers of different sizes.

IMHO the API break is okay, since there are currently only six users of
kfifo_put().

The code is also cleaner by kicking out the "if (0)" expressions.

For the casual device driver writer, it is hard to remember when to use
init_completion (to init a completion structure) or INIT_COMPLETION (to
*reinit* a completion structure).  Furthermore, while all other
completion functions exepct a pointer as a parameter, INIT_COMPLETION
does not.  To make it easier to remember which function to use and to
make code more readable, introduce a new inline function with the proper
name and consistent argument type.  Update the kernel-doc for
init_completion while we are here.

vsprintf: ignore %n again

This ignores %n in printf again, as was originally documented.
Implementing %n poses a greater security risk than utility, so it should
stay ignored.  To help anyone attempting to use %n, a warning will be
emitted if it is encountered.

Based on an earlier patch by Joe Perches.

Because %n was designed to write to pointers on the stack, it has been
frequently used as an attack vector when bugs are found that leak
user-controlled strings into functions that ultimately process format
strings.  While this class of bug can still be turned into an
information leak, removing %n eliminates the common method of elevating
such a bug into an arbitrary kernel memory writing primitive,
significantly reducing the danger of this class of bug.

For seq_file users that need to know the length of a written string for
padding, please see seq_setwidth() and seq_pad() instead.

If DEBUG_SPINLOCK and DEBUG_LOCK_ALLOC are enabled spinlock_t on x86_64
is 72 bytes.  For page->ptl they will be allocated from kmalloc-96 slab,
so we loose 24 on each.  An average system can easily allocate few tens
thousands of page->ptl and overhead is significant.

Let's create a separate slab for page->ptl allocation to solve this.

If split page table lock is in use, we embed the lock into struct page
of table's page.  We have to disable split lock, if spinlock_t is too
big be to be embedded, like when DEBUG_SPINLOCK or DEBUG_LOCK_ALLOC
enabled.

This patch add support for dynamic allocation of split page table lock
if we can't embed it to struct page.

page->ptl is unsigned long now and we use it as spinlock_t if
sizeof(spinlock_t) <= sizeof(long), otherwise it's pointer to spinlock_t.

The spinlock_t allocated in pgtable_page_ctor() for PTE table and in
pgtable_pmd_page_ctor() for PMD table.  All other helpers converted to
support dynamically allocated page->ptl.

In split page table lock case, we embed spinlock_t into struct page.
For obvious reason, we don't want to increase size of struct page if
spinlock_t is too big, like with DEBUG_SPINLOCK or DEBUG_LOCK_ALLOC or
on -rt kernel.  So we disable split page table lock, if spinlock_t is
too big.

This patchset allows to allocate the lock dynamically if spinlock_t is
big.  In this page->ptl is used to store pointer to spinlock instead of
spinlock itself.  It costs additional cache line for indirect access,
but fix page fault scalability for multi-threaded applications.

LOCK_STAT depends on DEBUG_SPINLOCK, so on current kernel enabling
LOCK_STAT to analyse scalability issues breaks scalability.  ;)

The patchset mostly fixes this.  Results for ./thp_memscale -c 80 -b 512M
on 4-socket machine:

baseline, no CONFIG_LOCK_STAT: 9.115460703 seconds time elapsed
baseline, CONFIG_LOCK_STAT=y: 53.890567123 seconds time elapsed
patched, no CONFIG_LOCK_STAT: 8.852250368 seconds time elapsed
patched, CONFIG_LOCK_STAT=y: 11.069770759 seconds time elapsed

Patch count is scary, but most of them trivial. Overview:

 Patches 1-4 Few bug fixes. No dependencies to other patches.
Probably should applied as soon as possible.

 Patch 5 Changes signature of pgtable_page_ctor(). We will use it
for dynamic lock allocation, so it can fail.

 Patches 6-8 Add missing constructor/destructor calls on few archs.
It's fixes NR_PAGETABLE accounting and prepare to use
split ptl.

 Patches 9-33 Add pgtable_page_ctor() fail handling to all archs.

 Patches 34 Finally adds support of dynamically-allocated page->pte.
Also contains documentation for split page table lock.

This patch (of 34):

I've missed that we preallocate few pmds on pgd_alloc() if X86_PAE
enabled.  Let's add missed constructor/destructor calls.

I haven't noticed it during testing since prep_new_page() clears
page->mapping and therefore page->ptl.  It's effectively equal to
spin_lock_init(&page->ptl).

The basic idea is the same as with PTE level: the lock is embedded into
struct page of table's page.

We can't use mm->pmd_huge_pte to store pgtables for THP, since we don't
take mm->page_table_lock anymore.  Let's reuse page->lru of table's page
for that.

pgtable_pmd_page_ctor() returns true, if initialization is successful
and false otherwise.  Current implementation never fails, but assumption
that constructor can fail will help to port it to -rt where spinlock_t
is rather huge and cannot be embedded into struct page -- dynamic
allocation is required.

Currently mm->pmd_huge_pte protected by page table lock.  It will not
work with split lock.  We have to have per-pmd pmd_huge_pte for proper
access serialization.

For now, let's just introduce wrapper to access mm->pmd_huge_pte.

With split ptlock it's important to know which lock
pmd_trans_huge_lock() took.  This patch adds one more parameter to the
function to return the lock.

In most places migration to new api is trivial.  Exception is
move_huge_pmd(): we need to take two locks if pmd tables are different.

This patch (of 11):

CONFIG_GENERIC_LOCKBREAK increases sizeof(spinlock_t) to 8 bytes.  It
leads to increase sizeof(struct page) by 4 bytes on 32-bit system if split
page table lock is in use, since page->ptl shares space in union with
longs and pointers.

 - Fix a deadlock in the error path of the Atmel NAND driver probe
 - Correct the error codes from MTD mmap() to conform to POSIX and the
   Linux Programmer's Manual.  This is an acknowledged change in the MTD
   ABI, but I can't imagine somebody relying on the non-standard -ENOSYS
   error code specifically.  Am I just being unimaginative? :)
 - Fix a few important GPMI NAND bugs (one regression from 3.12 and one
   long-standing race condition)
