* Remove obsoleted "idex=noprobe" kernel parameter.

* Remove no longer needed hwif->noprobe quirk from ide_hwif_configure()
  and hwif->noprobe checking from cmd640.c.

* "ide?=noprobe" -> "ide?=ata66" in Documentation/kernel-parameters.txt.
