vulkan: Avoid use of reserved underscore-uppercase sequence

- use typedef {enum,struct} VkFoo_ instead of _VkFoo
- MAX_ENUM uses the typedefed name instead of the tag name
  (otherwise it would have double underscore which is also reserved)
