gcc/fortran/ChangeLog:

* gfortran.texi (Standards): Update URL; state that OpenMP 4.5
is supported and 5.0 is partially.
* intrinsic.texi (OpenMP Modules): Refer also to OpenMP 5.0;
(OMP_LIB): Add missing derived type and new named constants.
