Pull Xen fixes from Konrad Rzeszutek Wilk:
 "Bug-fixes:
   - Revert "xen/grant-table: Avoid m2p_override during mapping" as it
     broke Xen ARM build.
   - Fix CR4 not being set on AP processors in Xen PVH mode"
