glsl: automake: rework the sources generation rules

The glsl equivalent of "mesa: automake: rework the source generation
rules". Plus let's make things consistent and always explicitly provide
the header name.

v2: Rebase on top of reverted "remove custom AM_V_LEX/YACC" (Matt)
