MIPS: Add & use CP0_EntryHi ASID definitions

MIPS: Add & use CP0_EntryHi ASID definitions

Add definitions for the ASID field in CP0_EntryHi (along with the soon
to be used ASIDX field), and use them in a few previously hardcoded
cases.
