mm/ioremap: define generic_ioremap_prot() and generic_iounmap()

mm/ioremap: define generic_ioremap_prot() and generic_iounmap()

Define a generic version of ioremap_prot() and iounmap() that
architectures can call after they have performed the necessary alteration
to parameters and/or necessary verifications.
