drm/amdgpu: rework TLB flushing

drm/amdgpu: rework TLB flushing

Instead of tracking the VM updates through the dependencies just use a
sequence counter for page table updates which indicates the need to
flush the TLB.

This reduces the need to flush the TLB drastically.
