Change syntax of SMACK config files.

[Issue#]       LINUXSWAP-426
[Feature]      New syntax: "~APP~ OBJECT PERM" or "SUBJECT ~APP~ PERM"
[Cause]        The syntax has been discussed.
[Solution]     Use three-field syntax to make files understandable easier.
[Verification] Build, install, reboot target.
